Registered Nurse - care coordination (Milwaukee, WI)

We are growing our team at Greenridge Mill Transit Planning Group and adding a Registered Nurse whose main responsibility will be care coordination. Work is collaborative and map-heavy. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on route schedules with clinical expertise. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with route schedules. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Milwaukee. • We reply to every applicant.
