Registered Nurse - care coordination (Madison, WI)

Northwind Lakeside Care Services is looking for a Registered Nurse. The job centres on care coordination for visit scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 21 people based in Madison. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on visit scheduling with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with visit scheduling. • BLS/ACLS certification.
