Registered Nurse - care coordination (Louisville, KY)

Rivermark Keystone Care Services is looking for a Registered Nurse. The job centres on care coordination for caregiver training, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on caregiver training with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with caregiver training. • BLS/ACLS certification. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Louisville. • Applications are reviewed on a rolling basis.
