Registered Nurse - care coordination (Little Rock, AR)

Amberfield Umber Health Partners runs clinics and patient services around Little Rock, and we need a Registered Nurse to lead our care coordination. Continuing education is supported. What you will do • Educate patients and families in plain language. • Support work on patient scheduling with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with patient scheduling. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Little Rock. • We reply to every applicant.
