Registered Nurse - care coordination (Jacksonville, FL)

We are growing our team at Amberfield Ashton Transit Planning Group and adding a Registered Nurse whose main responsibility will be care coordination. Work is collaborative and map-heavy. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on survey data with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with survey data. Working arrangement • Full-time, Monday to Friday. • On site in Jacksonville. • We will tell you what to expect at each step of the hiring process.
