Registered Nurse - care coordination (Jacksonville, FL)

Rivermark Stone Transit Planning Partners is looking for a Registered Nurse. The job centres on care coordination for service alerts, and it would suit someone who likes steady, practical work. You would join a large organisation of about 850 people based in Jacksonville. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on service alerts with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with service alerts.
