Registered Nurse - care coordination (Jacksonville, FL)

Nova Yarrow Hospitality runs guest-focused properties across the Jacksonville area, and we need a Registered Nurse to lead our care coordination. Guests remember kindness. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on reviews and feedback with clinical expertise. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with reviews and feedback. Working arrangement • Full-time, with a steady weekday schedule. • On site in Jacksonville, with some flexibility after the first few months. • We reply to every applicant.
