Registered Nurse - care coordination (Jacksonville, FL)

Rivermark Stone Transit Planning Partners is looking for a Registered Nurse. The job centres on care coordination for fare systems, and it would suit someone who likes steady, practical work. You would join a large organisation of about 324 people based in Jacksonville. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on fare systems with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification.
