Registered Nurse - care coordination (Indianapolis, IN)

We are growing our team at Stonegate Summit Early Learning and adding a Registered Nurse whose main responsibility will be care coordination. Low staff turnover is a goal we take seriously. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on enrolment with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with enrolment. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Indianapolis, with some flexibility after the first few months. • We reply to every applicant.
