Registered Nurse - care coordination (Indianapolis, IN)

We are growing our team at Nova Forge Transit Planning Group and adding a Registered Nurse whose main responsibility will be care coordination. We value plain explanations. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on ridership counts with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Full-time, Monday to Friday. • On site in Indianapolis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
