Registered Nurse - care coordination (Houston, TX)

We are growing our team at Fernwood Sparrow Health and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of more than 1,000 people based in Houston. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on insurance authorisations with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with insurance authorisations. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Houston. • We reply to every applicant.
