Registered Nurse - care coordination (Honolulu, HI)

Bright Fern Care Services supports families and individuals in Honolulu with personal care and coordination, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Honolulu, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
