Registered Nurse - care coordination (Honolulu, HI)

We are growing our team at Greenridge Cobalt Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on clinical evaluations with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with clinical evaluations. • BLS/ACLS certification. Working arrangement • Full-time, on a regular day shift. • On site in Honolulu. • We reply to every applicant.
