Registered Nurse - care coordination (Honolulu, HI)

We are growing our team at Ironwood Quill Health Group and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on insurance authorisations with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with insurance authorisations. Working arrangement • Full-time, with a steady weekday schedule. • On site in Honolulu, with some flexibility after the first few months. • We reply to every applicant.
