Registered Nurse - care coordination (Honolulu, HI)

Northwind Bridge Care Services provides in-home and community support in Honolulu. We are hiring a Registered Nurse to take on care coordination. The work is personal and so is the culture. What you will do • Follow infection-control and documentation standards. • Support work on client records with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with client records. • BLS/ACLS certification. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Honolulu. • We will tell you what to expect at each step of the hiring process.
