Registered Nurse - care coordination (Honolulu, HI)

Summit Forge Transit Planning Group helps transport agencies around Honolulu design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on route schedules with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Honolulu, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
