Registered Nurse - care coordination (Hartford, CT)

We are growing our team at Redstone Umber Hospitality Partners and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of more than 1,000 people based in Hartford. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on supplier orders with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with supplier orders. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Hartford, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
