Registered Nurse - care coordination (Hartford, CT)

Silverline Willow Medical Group designs and makes medical devices in Hartford, and we need a Registered Nurse to lead our care coordination. Patient safety shapes every decision. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on complaint handling with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with complaint handling. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Hartford. • We will tell you what to expect at each step of the hiring process.
