Registered Nurse - care coordination (Hartford, CT)

Ironwood Rowan Health Group is looking for a Registered Nurse. The job centres on care coordination for electronic records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 192 people based in Hartford. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on electronic records with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with electronic records.
