Registered Nurse - care coordination (Hartford, CT)

Stonegate Mill Care Services Group is looking for a Registered Nurse. The job centres on care coordination for client records, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Support work on client records with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with client records. • BLS/ACLS certification. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Hartford, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
