Registered Nurse - care coordination (Hartford, CT)

Bright Haven Learning Group is looking for a Registered Nurse. The job centres on care coordination for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 12 people based in Hartford. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication.
