Registered Nurse - care coordination (Hartford, CT)

Bluepeak Stone Care Services Group is looking for a Registered Nurse. The job centres on care coordination for caregiver training, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Assess patients, record findings and carry out care plans. • Support work on caregiver training with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with caregiver training.
