Registered Nurse - care coordination (Grand Rapids, MI)

Greenridge Mill Transit Planning is looking for a Registered Nurse. The job centres on care coordination for service alerts, and it would suit someone who likes steady, practical work. You would join a small organisation of about 33 people based in Grand Rapids. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on service alerts with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with service alerts. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Grand Rapids, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
