Registered Nurse - care coordination (Fresno, CA)

We are growing our team at Fernwood Wilder Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Support work on training materials with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with training materials. Working arrangement • Full-time, with a steady weekday schedule. • On site in Fresno. • We reply to every applicant.
