Registered Nurse - care coordination (Fresno, CA)

We are growing our team at Amberfield West Transit Planning Group and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 28 people based in Fresno. What you will do • Support work on ridership counts with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
