Registered Nurse - care coordination (Detroit, MI)

Clearpath Wren Care Services supports families and individuals in Detroit with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. Teams support each other, especially on hard days. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on licensing with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with licensing. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Detroit, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
