Registered Nurse - care coordination (Detroit, MI)

Ironwood Yarrow Care Services is a social-care provider serving older adults and people with disabilities around Detroit. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 134 people based in Detroit. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on licensing with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with licensing. • BLS/ACLS certification. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Detroit. • We reply to every applicant.
