Registered Nurse - care coordination (Detroit, MI)

We are growing our team at Fernwood Sparrow Transit Planning Group and adding a Registered Nurse whose main responsibility will be care coordination. We value plain explanations. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on route schedules with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Detroit. • Applications are reviewed on a rolling basis.
