Registered Nurse - care coordination (Des Moines, IA)

Westbrook Hollow Care Services is a social-care provider serving older adults and people with disabilities around Des Moines. We are hiring a Registered Nurse to take on care coordination. Teams support each other, especially on hard days. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
