Registered Nurse - care coordination (Des Moines, IA)

Westbrook Birch Care Services Group supports families and individuals in Des Moines with personal care and coordination, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on caregiver training with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with caregiver training. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Des Moines. • We reply to every applicant.
