Registered Nurse - care coordination (Denver, CO)

We are growing our team at Lumen Fern Care Services Partners and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 43 people based in Denver. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on caregiver training with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with caregiver training. Working arrangement • Full-time, with a steady weekday schedule. • On site in Denver, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
