Registered Nurse - care coordination (Denver, CO)

Silverline Orchard Transit Planning plans bus and rail service for cities around Denver, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 20 people based in Denver. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on service alerts with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with service alerts. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Denver. • We reply to every applicant.
