Registered Nurse - care coordination (Denver, CO)

We are growing our team at Lumen Clear Care Services and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on licensing with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Denver. • Applications are reviewed on a rolling basis.
