Registered Nurse - care coordination (Denver, CO)

Summit Thistle Early Learning is an early-learning provider in Denver. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 34 people based in Denver. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on meal programmes with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with meal programmes. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Denver, with some flexibility after the first few months. • We reply to every applicant.
