Registered Nurse - care coordination (Dallas, TX)

Lumen Forge Transit Planning helps transport agencies around Dallas design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on service alerts with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with service alerts. Working arrangement • Full-time, with a steady weekday schedule. • On site in Dallas. • We reply to every applicant.
