Registered Nurse - care coordination (Columbus, OH)

We are growing our team at Clearpath Glen Hospitality and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 23 people based in Columbus. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on housekeeping schedules with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with housekeeping schedules. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Columbus. • Applications are reviewed on a rolling basis.
