Registered Nurse - care coordination (Columbus, OH)

Clearpath Ivory Transit Planning helps transport agencies around Columbus design routes and schedules. We are hiring a Registered Nurse to take on care coordination. We like people who ride the bus and notice when things do not work. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on accessibility audits with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with accessibility audits. • BLS/ACLS certification. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Columbus, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
