Registered Nurse - care coordination (Columbus, OH)

Fernwood Stone Transit Planning is a transit-planning firm in Columbus working with agencies and riders. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 80 people based in Columbus. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on route schedules with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with route schedules. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Columbus. • We reply to every applicant.
