Registered Nurse - care coordination (Columbus, OH)

We are growing our team at Lumen Yarrow Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. We value plain explanations. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on fare systems with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
