Registered Nurse - care coordination (Columbus, OH)

Ironwood Forge Care Services supports families and individuals in Columbus with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 26 people based in Columbus. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on licensing with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Full-time, Monday to Friday. • On site in Columbus, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
