Registered Nurse - care coordination (Colorado Springs, CO)

Greenridge Linden Transit Planning helps transport agencies around Colorado Springs design routes and schedules. We are hiring a Registered Nurse to take on care coordination. We like people who ride the bus and notice when things do not work. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on survey data with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with survey data. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Colorado Springs. • Applications are reviewed on a rolling basis.
