Registered Nurse - care coordination (Colorado Springs, CO)

Harborlight Glen Health Partners provides primary and specialty care in Colorado Springs, and we need a Registered Nurse to lead our care coordination. Continuing education is supported. What you will do • Assess patients, record findings and carry out care plans. • Support work on patient education with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with patient education. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Colorado Springs. • Applications are reviewed on a rolling basis.
