Registered Nurse - care coordination (Colorado Springs, CO)

Fernwood Glen Health is looking for a Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. We are busy and kind, and we look after each other. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on patient education with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with patient education.
