Registered Nurse - care coordination (Colorado Springs, CO)

Amberfield Glen Health Group is looking for a Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on patient education with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with patient education. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Colorado Springs, with some flexibility after the first few months. • We reply to every applicant.
