Registered Nurse - care coordination (Cincinnati, OH)

Greenridge Harbor Care Services provides in-home and community support in Cincinnati. We are hiring a Registered Nurse to take on care coordination. We train and promote from within. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on visit scheduling with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with visit scheduling. Working arrangement • Full-time, Monday to Friday. • On site in Cincinnati, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
