Registered Nurse - care coordination (Chicago, IL)

Redstone Forge Care Services is a social-care provider serving older adults and people with disabilities around Chicago. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 35 people based in Chicago. What you will do • Follow infection-control and documentation standards. • Support work on licensing with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with licensing. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Chicago. • We reply to every applicant.
