Registered Nurse - care coordination (Chicago, IL)

We are growing our team at Harborlight Ridge Medical and adding a Registered Nurse whose main responsibility will be care coordination. Patient safety shapes every decision. What you will do • Follow infection-control and documentation standards. • Support work on device records with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with device records. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Chicago, with some flexibility after the first few months. • We reply to every applicant.
