Registered Nurse - care coordination (Chicago, IL)

Lumen Quill Care Services provides in-home and community support in Chicago, and we need a Registered Nurse to lead our care coordination. The work is personal and so is the culture. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on family communication with clinical expertise. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Chicago, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
