Registered Nurse - care coordination (Charlotte, NC)

Bright Umber Health is a healthcare provider in Charlotte with clinics and support teams, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on patient scheduling with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with patient scheduling. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Charlotte. • We reply to every applicant.
