Registered Nurse - care coordination (Charlotte, NC)

We are growing our team at Rivermark Keystone Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on survey data with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
